Basically - pick the melody note with your thumb, and then pick the three single-pitch notes with your ring-middle-index fingers; the technique is called tremelo, and it's difficult to play without it sounding like a galloping horse.

Complex arrangement is not what makes a piece easy or hard, playing to perfection is.
